What is your new patient procedure?
New patients should first book their appointment by choosing the service that best suits their needs. Once the appointment is booked, the patient will need to go to the Forms page to fill out the New Patient Assessment Form. If possible, this form should be submitted to the provider at least 24 hours prior to the appointment. Once the New Patient Assessment Form has been submitted, patients should just log on through the secure link sent via email.
I'm an established patient. Is there anything I need to do to for my upcoming appointment?
New patients will be requested to complete a Release of Information Form to help our provider in your plan of care. You may print the form out, complete it and then take it to your primary care provider so your medical records can be requested for your upcoming appointment.

What if I don't receive a confirmation for my appointment or an email with a link for my appointment?

Is telehealth a good option for my needs?
There were many learning opportunities when COVID hit in 2020. One opportunity was realizing that Telehealth is a good option for routine assessments. With being face to face and one to one, even on the "tele" is an approved method of healthcare within IL. If during your assessment it is determined that you may need more acute attention, we will be happy to help walk you through the process.
How can I submit this for payment through my health insurance?
After your appointment, you will be emailed a copy of your visit's receipt. This receipt will have the information most insurance companies require if you want to submit your receipt for payment. If you have any questions, please speak to your health insurance representative before your appointment to ensure there are no issues with your reimbursement.
